PDHI Adds Diabetes and Heart Disease Prevention to ConXus Steps Library of Action Plans

Like their predecessors in the ConXus Steps library, each new action plan comprises:
- A pre- and post-assessment of disease risk using biometrics data and self-reporting of lifestyle behaviors
- An interactive tutorial on disease prevention
- Eight learning steps on improving physical activity, eating habits, early detection of disease through testing, and compliance with the program. In addition, the heart disease prevention plan offers a lesson on medication use.
- Weekly progress trackers
- Integrated coaching sessions for clients using the ConXus Direct health coaching module
- An optional satisfaction survey
To increase engagement, each action plan supports reminders for participants to complete a step. Participants may earn incentive points and a bonus for completing all steps within a plan.

The ConXus Steps library also includes action plans for healthy eating, weight management, exercise and activity, stress management, and smoking cessation.
ConXus Steps has received Health Information Products (HIP) 2 Certification from the National Committee for Quality Assurance (NCQA).
All the health action plans can be configured to match client service levels and business rules, including program duration, action steps included, and incentive points available. Integration with ConXus Rewards provides comprehensive tracking and fulfillment of incentives.
ConXus Steps action plans are an integral component of the ConXus platform. Designed for organizations that deliver health and wellness programs to multiple employer end clients, the platform enables assessment and stratification, biometric screening data integration, behavior change reporting, health coaching, wellness challenges and events, and incentive tracking and fulfillment.
ConXus Steps also supports online wellness programs in the form of:

ConXus Steps supports a self-directed model for personal health improvement or a higher-touch, coach-directed model when combined with ConXus Direct.
