Health Diagnostic Laboratory, Inc. Partners with Patient-Centered Care Collaborative
RICHMOND, Va., Oct. 13, 2011 /PRNewswire/ — Health Diagnostic Laboratory Inc. (HDL, Inc.), a CAP accredited leader in health management, announced today that it is the first advanced laboratory to join the Patient-Centered Primary Care Collaborative (PCPCC) executive committee. The PCPCC, created in late 2006, is one of the major developers and advocates of the patient centered medical home (PCMH) model in America. The Collaborative membership includes a number of large national employers, major primary care physician associations, health benefits companies, trade associations, profession/affinity groups, academic centers, and healthcare quality improvement associations.
PCMH’s approach incorporates the whole person and self-care, where patients with chronic health conditions, like cardiovascular disease, are proactively supported in making lifestyle changes for prevention and wellness rather than being treated for symptoms of disease.
“Healthcare reform includes a renewed emphasis on the patient-centered medical home model, where disease management and prevention and wellness are core concepts,” said Tonya Mallory, CEO and co-founder of HDL, Inc. “Advanced testing and customized disease management tools offered by Health Diagnostic Laboratory, Inc. — including personal health plans, customized dietary and exercise plans, and personal health coaching — supports the PCMH model of pro-active healthcare in a physician’s practice.”
HDL, Inc. will partner with PCPCC and participate in its open forums where healthcare stakeholders freely communicate and work together to improve the future of the American medical system. The two organizations will strive toward a common goal of continued education of the PCHM model as a superior form of healthcare delivery.
In addition to joining the PCPCC executive committee, HDL, Inc. will serve as sponsor of the 5th Annual Patient-Centered Primary Care Collaborative Summit in support of its mission to improve care in the PCMH. Hosted on October 21, 2011 in Washington, DC, the event promises to celebrate five years of making healthy connections.
“The PCMH model emphasizes the effective management of high risk and chronic disease,” said Mallory. “HDL, Inc. provides advanced testing and personal health coaching services that are critical to successful implementation of health promotion and prevention initiatives within a physician’s practice to enable the PCMH approach for patient care.”
