North Dakota Department of Human Services, Medical Services Division Selects U.S. Care Management to Provide New Medicaid Care Management Program

JACKSONVILLE, Fla., Nov. 27 /PRNewswire/ -- U.S. Care Management (formerly Specialty Disease Management) announced today it has been selected by the North Dakota Department of Human Services to provide community-based disease management services to help address the complex health care needs of eligible Medicaid recipients with chronic health conditions living in the state.
Experience HealthND is being implemented in cooperation with the University of North Dakota's School of Medicine and Health Sciences and College of Nursing who will provide continuing education for providers and augment outreach and information services for recipients. The Program will implement innovative approaches to providing disease and care management services for Medicaid fee-for-service recipients who have one or more of the following chronic illnesses including asthma, congestive heart failure, chronic obstructive pulmonary disease (COPD) and diabetes.
U.S. Care Management, a wholly-owned subsidiary of U.S. Preventive Medicine, will manage a team of RN Care Managers who live and work in the state. Nurses will provide face-to-face and telephonic assistance to recipients offering care coordination, education and monitoring services to improve self-care, adherence to their doctor's treatment plans and understanding of available services and treatment options. A 24-hour, seven day a week toll free nurse phone line will be available for recipients to access information.
Hoadley H. Harris, MD, will serve as the Medical Advisor for Experience HealthND overseeing the Program's State Physician Advisory Committee that provides recommendations regarding administration and development of the Program.
