VALLEJO, Calif., Jan. 14 /PRNewswire/ -- Sutter Solano Medical Center is proud to announce that Richard Jacobs, M.D., MBA, vice president of Medical Affairs, has been named the recipient of the prestigious Windsor 2007 Physician of the Year Award presented by The Windsor Family of Health Care Facilities. Dr. Jacobs was recognized for his exceptional work and collaboration with Vallejo Care Center, one of the hospital's community partners that care for long-term care patients.
Dr. Jacobs is no stranger to the long-term care business. In fact, he served as a past board member and treasurer of The American Medical Directors Association, a 6,000-physician member professional association of medical directors and physicians dedicated to excellence in patient care by providing education, advocacy, information and professional development. When he joined Sutter Solano Medical Center two years ago, it was a natural step for him to identify the importance of continuum-of-care and liaison work with Vallejo Care Center.
"I'm honored to receive this recognition, but this was only made possible through collaborative efforts of the nursing team, case management staff and hospitalists," notes Dr. Jacobs. "The great relationship we've built with Vallejo Care Center has allowed us to greatly improve patient safety and care, planning for the future care of our patients, and the transitions between hospital and skilled care."
The Windsor 2007 Physician of the Year Award was presented with a $4,000 recognition "reward." Dr. Jacobs has graciously asked that the value of the award be donated to Sutter Solano Charitable Foundation, a philanthropic organization that provides funds for needed but otherwise unfunded medical services in Solano County.
"We're proud to partner with physicians such as Dr. Jacobs," says Mr. Lee C. Samson, president of The Windsor Family of Health Care Facilities. "He truly makes a valued difference at Vallejo Care Center-this award is a testament to his hard work."
