Health Dialog Selected to Provide Health and Wellness Support to 17,000 Medicare Beneficiaries

Health Dialog Services Corporation today announced it has been selected by the Centers for Medicare and Medicaid Services (CMS) to provide health and wellness support to more than 17,000 Medicare fee-for-service beneficiaries as part of the CMS Senior Risk Reduction Demonstration (SRRD).

Under the three-year agreement, 17,000 beneficiaries from across the country between the ages of 67 and 74 will have access to Health Dialog's "Healthy Living Solutions for Seniors" program, which includes telephonic Health Coaching and related services. The "Healthy Living Solutions for Seniors" program is designed to motivate individuals to become more actively involved in the management of their health. Research shows that the program improves the health status of the individuals involved and reduces health care costs. As part of the program, Health Dialog Health Coaches, who are registered nurses, dieticians, and respiratory therapists, will provide beneficiaries with evidence-based, unbiased information about their conditions and health status so they can work more effectively and confidently with their physicians. Further, the program will offer access to a wealth of healthcare support tools, including a Personal Health Assessment tool, Shared Decision-Making® decision support videos, and a broad set of online multi-media behavior change modules provided in collaboration with Health Media. Health Dialog also plans to include a scientifically validated online behavior change program to increase physical activity, created by researchers at the Oregon Center for Applied Science, Inc. (ORCAS).
