On-Site Primary and Preventive Care for Nursing-Home Residents in Los Angeles and Orange Counties Offered By PacifiCare Health Systems and Talbert Medical Group

CYPRESS, Calif., April 6 /PRNewswire-FirstCall/ -- PacifiCare Health Systems Inc. is teaming with Talbert Medical Group to bring primary and preventive medical care directly to nursing-home residents in Los Angeles and Orange counties through the SecureAdvantage from PacifiCare(SM) program.
Designed to enhance the overall health and well-being of nursing-home residents and reduce emergency-room visits and preventable hospitalizations, SecureAdvantage from PacifiCare offers primary and preventive medical care onsite to nursing-home residents.
SecureAdvantage from PacifiCare is currently available to Secure Horizons Medicare Advantage members who reside in select nursing facilities in Los Angeles and Orange counties and who choose a Talbert Medical Group doctor as his/her primary care physician.
Through SecureAdvantage from PacifiCare, Talbert Medical Group physicians and nurse practitioners are able to visit nursing-home residents more frequently and provide an enhanced level of preventive and chronic care management directly to patients, according to Kathy Feeny, PacifiCare's executive vice president of Secure Horizons Senior Solutions.

University of Minnesota researchers evaluated similar programs that operated as federal demonstration projects over the past decade. The research found that these programs were effective in providing quality, coordinated care for nursing-home residents while reducing hospital admissions.
