Qtrac Software Signs Partnership With Patient Placement Systems

Qtrac Software and Patient Placement Systems, a provider of patient referral software, have announced a partnership to help long term care facilities efficiently manage patient referrals and intake and seamlessly transition to an electronic health record designed to deliver quality of care.
Through this partnership, Patient Placement Systems will connect disparate referral, admissions and discharge databases with Qtrac's clinical care applications to streamline communication, transfer of data and enable new residents to be quickly and efficiently transitioned into a managed care delivery model with supporting documentation.
This partnership is expected to provide an opportunity to develop innovative solutions that facilitate an efficient and accurate integration of patient referral, admission, document management and care delivery.
