Cielo MedSolutions Signs Business Affiliate Agreement With TransforMED
Cielo MedSolutions, a provider of healthcare software and web applications, has signed a preferred business affiliate agreement with TransforMED, a wholly owned subsidiary of the American Academy of Family Physicians.
Through this relationship, Cielo MedSolutions’s customers will have access to TransforMED’s practice transformation services. In addition, TransforMED will encourage medical practices implementing a patient-centered medical home to use Cielo Clinic’s integrated patient registry, clinical decision support and population management tools.
According to Cielo MedSolutions, TransforMED provides practical support and consulting services to primary care physicians and health systems that want to transform their medical practices into patient-centered medical homes. The TransforMED medical home model combines the relationship-centered values of primary care with new technologies to improve quality of care and efficiency, as well as patient and physician satisfaction. The organization made the decision to join with Cielo MedSolutions because of its proven track record for improving the coordination of care.
Terry McGeeney, president and CEO of TransforMED, said: “As Patient-Centered Medical Homes, primary care practices serve as the focal point of patient care coordination across all specialties. Our goal is to identify technologies that can assist practices to improve diagnostic capabilities and to enhance treatment and outcomes. We are confident that this relationship will have a positive impact on both patients and physicians.”
Dave Morin, president and CEO of Cielo MedSolutions, said: “We at Cielo share TransforMED’s firm conviction in the patient-centered medical home. Fundamental to a successful medical home implementation is the robust functionality of Cielo Clinic and a trained and engaged practice team, which TransforMED can help build. The combination of TransforMED’s services and our Cielo Clinic technology will allow practices to achieve a successful and transformative medical home.”
