Health Integrated Achieves NCQA Full Accreditation for Its Synergy Targeted Population Management(TM) Program

TAMPA, Fla., Jan. 29 /PRNewswire/ -- Health Integrated, a Targeted Population Health Management company focused on the critical interplay between medical and psychosocial health, has achieved Full Accreditation for Disease Management by the National Committee for Quality Assurance (NCQA). Health Integrated received the accreditation for its Synergy Targeted Population Management(TM) program in seven chronic disease areas: asthma, chronic obstructive pulmonary disease, congestive heart failure, coronary artery disease, depression, diabetes and low back pain.

The NCQA awards its highest status of Full Accreditation to organizations that meet or exceed its stringent standards.
