Portable Health Care Records Now Available for Consumers

OAKLAND, Calif., Aug. 5 /PRNewswire/ -- Anyone can now easily create a personal and portable health care record in a standardized format understood at hospitals, clinics and doctors' offices. Heartwood Health, a health care management company specializing in chronic illness support, geriatric care management and private health care advocacy, has announced a partnership agreement with Chart Your Own Health to re-brand and sell the popular My Health Care Record medical documentation system.

Heartwood Health Private Health Record is a comprehensive health records organizer not unlike the familiar and easy to use time management organizers that have been popular in business for decades. The organizer is divided into tabs and forms and can even track personal data over time, such as weight or blood pressure readings. Because it uses the paper organizer paradigm - familiar to almost everyone - it is "elder friendly," and requires no computers or complicated software, which can be so hard to learn for many of the elderly.
