All Eyes on The Medical Home: Patient-Centered Primary Care Collaborative Convenes National Summit In Washington, DC

Third annual PCPCC Summit will spotlight converging goals of reform and health care delivery transformation via the patient centered medical home
WASHINGTON, Sept. 1 /PRNewswire-USNewswire/ -- As legislators on both sides of the aisle hotly debate how to extend health care coverage to more Americans and curb spiraling costs, advocates for a patient-centered model of care will converge on Washington, DC to make their case for a fundamentally improved means to deliver health care with better quality outcomes and higher patient satisfaction at a lower cost.
The Third Annual Patient-Centered Primary Care Collaborative Summit will convene Thursday, Oct. 22 at the Washington Convention Center. The Summit theme, "All Eyes on the Patient Centered Medical Home" acknowledges the central role the patient PCMH model has garnered in the health reform debate. Hundreds of health care stakeholders--providers, employer/purchasers, suppliers and policymakers--will unite around the Summit's goal to build, expand and support the PCMH model.
"While health care reform is the top concern in Washington this fall, the PCMH goes beyond ensuring coverage to improving the way patients care is delivered," said Edwina Rogers, executive director of the PCPCC. "The PCMH encourages people to select a primary care clinician who can give them comprehensive health care--from wellness and prevention to ongoing care for chronic conditions such as diabetes. The PCMH puts the patient at the center of continuous, uninterrupted care, coordinated in partnership with a personal provider with the right tools that will lead to better health."
The Patient-Centered Primary Care Collaborative (PCPCC) is a coalition of more than 600 organizations representing the nation's business leaders, consumer groups, organizations representing primary care physicians and other health care stakeholders. The PCMH movement aims to partner every American with a primary health care provider who is responsible and accountable for ongoing care and care coordination.
During the Summit, health policy experts will present the latest information on the role the PCMH plays in health reform efforts, as well as the impact it already is making within public initiatives on both the state and national levels. Representatives from the White House and key lawmakers have been invited to address the PCMH within reform efforts. In other sessions, academic experts, consumer advocates and doctors already using the PCMH model will share best practices already proven in communities via demonstration projects and pilot programs across the country. Evidence from these pilot programs has demonstrated improved health outcomes as well as real cost savings by reducing the number of unnecessary tests, consultations and emergency room visits.

The PCPCC is organized and financed to provide better outcomes for patients, more efficient payment to physicians and better value, accountability and transparency to purchasers and consumers.
