ANAHEIM, Calif., Oct. 28 /PRNewswire/ -- Crescent Healthcare has signed a strategic partnership agreement with Providence Health & Services, California to implement its Medical Management Solutions (MMS) at three of Providence's Los Angeles area hospitals. The new partnership will coordinate care for Providence's infusion patient population to assist in facilitating the best clinical and financial outcomes.
The two organizations will provide the community with an innovative approach to continuum of care by deploying the program at the three medical centers in the San Fernando Valley: Providence Saint Joseph, Providence Holy Cross, and Providence Tarzana medical centers. Crescent will place specially-trained clinicians on site to identify patients for early discharge and assist in the transition to an alternate setting.
Dave Massey, Senior Vice President of Acute Therapies for Crescent Healthcare, said several new Crescent Ambulatory Treatment Center (ATC) locations have been established to also serve Providence patients.
"We are opening three ATCs to facilitate the efficient delivery of infusion therapies to Providence's patient population," Massey said. "Patients will be able to schedule appointments at their convenience. Infusion certified nurses will monitor infusions, provide patient education, and answer questions, improving patient adherence to prescribed treatments."
The deployment of Crescent's Medical Management Solutions (MMS) has delivered more than $1 million in health care cost savings to Crescent's strategic partners in 2009. Robert Funari, Chairman and CEO of Crescent Healthcare, believes Crescent's Medical Management Solutions will strengthen collaboration between hospitals and outpatient care organizations. The goal is cost-effective delivery of complex drug therapy that reduces hospital stays.
"We have had significant success in assisting hospitals with hospital patient throughput," Funari stated. "We look forward to applying our methodologies to Providence's specific challenges to reduce costs at their Valley hospitals while providing smooth transitions into ambulatory care for their patients."
