Chronic Disease Management in Primary Care - Quality and Outcomes

This text is a timely contribution to the challenge of managing chronic disease in primary care and in terms of the use of problem- based learning. It offers a useful vehicle for the development of team-working, problem-solving skills and the adoption of evidence- based practice.

The introduction of the General Medical Services contract focuses on the ten clinical fields for which there are quality indicators. There is a useful example on diabetes care that demonstrates the process of learning in action. Topics include interprofessional learning, assessing learning and service development needs, diabetes, hypertension, hypothyroid disease, asthma, mental health, coronary heart disease, cancer and palliative care. There are also chapters on patient safety and medicine management.

This is perhaps best used as a 'how to do it' reference for the entire primary care team. It comes with a CD-ROM to assist the team in using ready-made recording forms or role and responsibility checklists. This is an excellent addition to the literature in this field.
