CMS Prepares for Chronic-Care Management System

The Centers for Medicare and Medicaid Services has released a draft statement of work for requirements for its Chronic Care Improvement Information Management System.
The program will be designed to improve the quality of clinical care for Medicare beneficiaries with one or more conditions, initially focusing on those with congestive heart failure and/or diabetes, according to CMS, an agency of the Health and Human Services Department, in its release.
The new system, which would provide for disease management pilot programs, will support the existing chronic-care improvement system that stores claims, eligibility/enrollment, participation, audit, payment, performance and other data. That system, which is a relational database, resides on the Medicare Data Communications Network.
The chronic-care system will support providers, chronic-care improvement evaluators and Medicare health support organizations. It also aims to help meet spending targets for Medicare beneficiaries with those conditions.
Comments are due Nov. 25. CMS is expected to release its request for proposals on or about Nov. 28.
