Capital District Physicians' Health Plan Selects Health Dialog to Provide Care Management Services

BOSTON, Mass., Dec. 12 /PRNewswire/ -- Health Dialog Services Corporation today announced that it has been selected by Capital District Physicians' Health Plan (CDPHP) to provide 212,000 CDPHP members and their dependents access to whole person care management services, including disease management. CDPHP is a physician-directed health plan serving New York state residents.
Through this agreement, eligible individuals from CDPHP's Commercial, Medicare, and Medicaid populations will have access to Health Coaching support for chronic conditions, including coronary artery disease, congestive heart failure, chronic obstructive pulmonary disease, and diabetes. Additionally, as part of Health Dialog's whole person approach to care management, individuals can obtain information and support across the spectrum of healthcare needs, including support for significant medical decisions and general health and wellness concerns.
Health Dialog Health Coaches work with individuals using a Shared Decision-Making(R) approach, developed in collaboration with the Foundation for Informed Medical Decision-Making (FIMDM), which encourages skill transfer and self-reliance throughout the Health Coaching experience. Health Coaches are specially trained healthcare professionals, such as nurses and dieticians, who help individuals become more active and effective participants in the management of their health.
Founded and guided by physicians, CDPHP and its affiliates currently serve members in 29 counties throughout New York state and seven counties in Vermont.
