CHICAGO, Feb. 15, 2006 (PRIMEZONE) -- Aveta Health Illinois, Inc. (AHI), a provider of Medicare Advantage services, has launched Aveta CarePartners, a Medicare Advantage Special Needs Plan (SNP) specially designed for persons with certain chronic illnesses. Effective January 1, 2006, AHI's "Aveta CarePartners" is available to Medicare beneficiaries in Cook County diagnosed with chronic illnesses, including chronic obstructive pulmonary disease (COPD); pre-end stage renal disease (kidney failure) or chronic heart disease, including congestive heart failure (CHF). Nationwide, AHI is one of only 11 organizations authorized by the Centers for Medicare and Medicaid Services (CMS) to offer SNPs for the chronically ill.
Aveta CarePartners' unique approach is based on a relationship between the beneficiary, a physician specializing in the treatment of their chronic condition, and a dedicated CarePartner to develop personal treatment plans and tailored lifestyle improvements to ensure quality care and to minimize the need for hospitalization. The Medicare Advantage SNP also includes Part D Medicare prescription drug coverage and provides more benefits than traditional Medicare coverage, including zero premiums and lower co-pays, deductibles and out-of-pocket limits.
"Aveta CarePartners empowers beneficiaries to take control of their chronic illness and play an active role in improving their health and quality of life," said Mr. Wayne Aardsma, President and CEO of AHI. "By focusing our approach, resources and expertise on specific chronic illnesses, Aveta CarePartners ensures that chronically ill beneficiaries receive the care they need while keeping healthcare costs affordable."
