Fallon Community Health Plan Teams With Health Dialog to Offer Care Management Services; Plan Members Will Have 24/7 Access to Professional Health Coaches

Health Dialog Services Corporation today announced that it has been selected by Fallon Community Health Plan (FCHP) to provide FCHP's members access to care management services, including 24/7 Health Coaching support across a spectrum of healthcare needs.

Through this new service, Health Dialog Health Coaches, who are nurses, dieticians, and respiratory therapists, will work directly with FCHP members both telephonically and online to help them manage chronic conditions, significant medical decisions, and general health and wellness concerns. Using a Shared Decision-Making(R) approach, Health Coaches are trained to help individuals understand the risks, benefits, and outcomes of various treatment options so they can gain the skills and self-reliance necessary to work more effectively with their physicians.
