Pennsylvania Makes New Resource Available to Young People With Special Health Care Needs

HARRISBURG, Pa., April 12 /PRNewswire/ -- Young adults with special health care needs will be able to better transition from youth to adult services when the Transition Health Care Checklist goes active on the Pennsylvania Department of Health's Web site today.
When children with special health care needs turn 18, their health care and prescription drug coverage often changes. Health care providers may change from pediatric specialists to adult medical specialists. The Transition Health Care Checklist can help young adults, families and professionals in navigating these changes.
The checklist, which is part of the department's efforts to address identified gaps in health services, provides a variety of resources, including a flow chart of health insurance options; a transition time line; and information and links to organizations that may be able to provide assistance such as Medicaid, Children's Health Insurance Plan, or CHIP, and adultBasic, which is a limited health insurance plan for Pennsylvanians.
The departments of Health, Education, Labor & Industry, and Public Welfare worked together to compile and publish this checklist.
