Often under-diagnosed and under-treated, chronic obstructive pulmonary disease (COPD) is usually not diagnosed in patients until they incur significant lung damage. With approximately 15 to 20% of all smokers eventually developing COPD, there are over 26 million patients worldwide who suffer from the disease, yet, only about 600,000 are diagnosed each year. It is estimated that less than half of Americans with lung function impairment have been properly diagnosed. Primary care physicians are being encouraged to routinely perform spirometry tests on smokers older than 45 years, which will increase diagnosis rates and drive market growth through 2010. Recently released bronchodilators Advair (salmeterol/fluticasone) and Spiriva (tiotropium bromide) have improved the standard of care for COPD. While most therapeutics emerging by 2010 are either reformulations of current drugs or products currently marketed for asthma, there are two, cilomilast (Ariflo) and roflumilast (Daxas), that comprise a novel class of phosphodiesterase 4 (PDE 4) inhibitors. These drugs offer oral tablet formulations, once or twice daily dosing, and both inflammatory and bronchodilator properties, and are expected to change the standard of care and significantly drive market growth. The COPD market is, which hold 84% of the market as of 2005. Although these players will remain the leaders in 2010, they will lose some market share to ALTANA Pharma, AstraZeneca, Sepracor, and Pfizer.
