Health Hero Network and American Medical Group Association Form Strategic Alliance to Improve Chronic Care Management

Health Hero Network, a leading innovator of technology solutions for chronic care management, announced it has formed a strategic alliance with the American Medical Group Association (AMGA). The two organizations will work together to develop best practice protocols and operating procedures for medical groups to follow when implementing care-management programs supported by technology-based solutions, such as the Health Buddy(R) system. Additionally, AMGA will initiate programs designed to facilitate dialogue among AMGA members to disseminate best practice learnings about technology-based management solutions and to pave the way for future chronic care management advancements.

The Health Buddy system enables physician groups to implement high quality care management programs. The Health Hero Network and AMGA partnership is a key initiative designed to address the challenges in chronic care management. The community will meet annually to develop and review formalized care management process for different disease states and will serve as an information network for improved care management. Other elements of the partnership include bimonthly webinars.
