The Philippines is one of the poorest countries of south east Asia. Impressive pre-crisis growth rates created a relatively wealthy middle class in the Manila/Quezon City region, and it is here that the best medical facilities are to be found. Elsewhere, the health situation of the population remains poor. Localised but severe outbreaks of infectious diseases, such as cholera, are frequent, and access to adequate healthcare in poor areas remains limited.
Hospitals often lack sufficient equipment, and in any case charge too much for many people to use. Many prefer not to visit hospitals, due to the costs involved, although, as in Taiwan, this may change if the current insurance reforms prove a success. Public hospitals are often the only option; a 1992 Department of Health survey found that around 75% of households were reliant on public hospitals, despite the public sector only supplying around 50% of the country's bed capacity. As public hospitals charge for treatment, those without insurance cover often prefer to do without.
Many hospitals, especially outside the prestigious facilities in Manila, lack adequate equipment. Limited budgets mainly have to be spent on personnel and recurrent items, leaving little room for smaller facilities to improve their stock of medical equipment. The management of hospital waste has been a major concern in recent years. Many hospitals lack adequate waste disposal procedures, burying untreated waste at shallow landfill sites, or in some cases disposing of it in nearby rivers. In Metro Manila, incineration facilities have been improved, although the situation in the rest of the country remains a cause for concern.
