Senior Whole Health Expands Corporate Footprint: Launches Special Needs Plans for Medicare/Medicaid Dual Eligibles in Connecticut, Replicating Successes in MA and NY

Posted on: Monday, 11 December 2006, 12:01 CST

Senior Whole Health, a leading health care plan providing intensive, integrated case management services to seniors 65 and older, today announced the launch of its portfolio of Special Needs Plans (SNPs) to Medicare/Medicaid dual eligible and Part D beneficiaries in Connecticut's Hartford, New Haven and Middlesex counties. Senior Whole Health currently serves 50 percent of participating dual eligible beneficiaries in Massachusetts and is enrolling qualified individuals in New York, spanning Albany, Dutchess, Saratoga, Schenectady, Rensselaer, and Ulster Counties.

"People who were always last in line for quality health care services now find themselves at the head of the queue, with access to a Senior Whole Health Registered Nurse Coordinator who advocates for members and directs each individual's medical and social services with providers," says John Baackes, CEO of Senior Whole Health, which coordinates comprehensive services including preventive, primary, acute and specialty medical care, behavioral health, extended care, home care, community-based services and pharmacy. "This personalized attention helps members, their families and caregivers to successfully navigate the health care system, relieving the stress associated with attempts to locate, arrange and monitor care."

The company's successful track record is also the result of positive relationships with medical providers in each state.

"Senior Whole Health is a leading example of how managed care is supposed to work -- it is centered around the patient," explains Kathleen Bennett, M.D., Medical Director, Senior Whole Health. "The clinical services team is the patient's advocate for meeting medical, psychological and social needs."

For low-income, dual eligible seniors, Senior Whole Health is the gold standard for addressing the current fragmentation of care delivery systems, with one entity streamlining the coordination of all facets of well being -- clinical, functional, psychosocial and long term. A key component of the program is identification of members at high risk through comprehensive assessment tools. These services provide a valuable resource to both providers and patients, improving the care delivered to each individual and enhancing the quality of life for seniors and their caregivers.
