Pfizer Health Solutions Selects D2Hawkeye to Provide Risk Stratification to Florida State Medicaid Project

D2Hawkeye, a leader in healthcare data analytics, has been selected by Pfizer Health Solutions Inc (PHS) to provide risk stratification services for the HealthierFlorida disease management program. In addition, D2Hawkeye will be providing care analysis and other metrics to the clinical outreach team. Under the terms of its contract with PHS, D2Hawkeye will employ state-of-the-art risk modeling tools to identify Florida Medicaid participants at risk for health complications due to chronic conditions and individuals who may benefit from better care coordination.

D2Hawkeye currently provides the data analytics for other state disease management programs, such as the State of Alabama. PHS was recently awarded the two-year contract to provide disease management services for the HealthierFlorida program. This community-based program is designed to unite hospitals, nurse care managers, nurse call centers and triage lines throughout Florida to help more than 90,000 Medicaid beneficiaries who suffer from chronic diseases such as congestive heart failure, chronic obstructive pulmonary disease , end stage renal disease, sickle cell anemia, diabetes, asthma and hypertension.
