AmeriHealth New Jersey Wins Disease Management Association of America Award for Innovative Programs

MT. LAUREL, N.J., Feb. 7 /PRNewswire/ -- The Disease Management Association of America (DMAA) has awarded AmeriHealth New Jersey the 2006 Disease Management Leadership Award for Outstanding Health Plan, in recognition of the company's Connections(SM) disease management program that helps members with chronic and other conditions better manage their health. Henry Schwerner, MD, associate medical director at AmeriHealth New Jersey, accepted the award at the DMAA's seventh annual Disease Management Leadership Forum in Denver, Colorado.
Each year, the award is given to recognize the health plan in America that best demonstrates vision and leadership in its commitment to disease management, innovative program design, successful plan implementation, and outstanding performance in measuring positive, proven health outcomes.
An evaluation of first-year results completed earlier this year showed that AmeriHealth's Connections disease management program lowered medical cost trends by 1.5 to 2 percent for the five most common chronic diseases: asthma, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), coronary artery disease (CAD), and diabetes. Preliminary second-year results are equally positive.

AmeriHealth began its current disease management program in 2002 to help members with complex chronic diseases. Connections has expanded to support 22 common and complex chronic conditions - among the most offered by any health plan - including end-stage renal disease, multiple sclerosis, Parkinson's, rheumatoid arthritis, and the recently added Crohn's disease. The Connections programs are available to approximately 215,000 AmeriHealth plan members, including more than 26,000 AmeriHealth members with at least one of the five most common chronic illnesses.
