Jewel-Osco Pharmacy Helps to Vitalize Members of The Destiny Health Plan

Jewel-Osco Pharmacy, a leading network of pharmacies providing patient care services in the Chicagoland area, has entered into an agreement with Destiny Health, a pioneer of the consumer-directed health care model, to provide a wellness service, Vitality Check™, to The Destiny Health Plan members.

A Vitality Check is a health assessment that members are encouraged to partake in to learn more about their current health condition. For example, during a Vitality Check, Jewel-Osco pharmacists will test member's blood glucose, blood pressure and cholesterol levels, as well as obtain height and weight measurements to calculate Body Mass Index (BMI). These results will be sent directly to The Destiny Health Plan, and members will be awarded Vitality Bucks™ for participating in the Vitality Check and for the test results as well, depending on how favorable the outcome is. The closer they are to being in the healthy range, the higher the Bucks received.

Fully integrated with the insurance and health savings account portions of The Destiny Health Plan, Vitality is a wellness program that encourages members with tangible rewards. Destiny Health Plan members can redeem their Vitality Bucks for Vitality Rewards® that include airline miles, vacation packages, name brand merchandise, iTunes and more.

Participating Jewel-Osco pharmacy locations will charge members only $40 for a Vitality Check, and if they have a Preferred Card, they will receive an additional $5 discount. Members will have their blood tested and their results back within minutes of the procedure.
