Physicians Still Treating Acute Otitis Media (AOM) As They Did Prior to PCV-7

ASHEVILLE, N.C., Feb. 26 /PRNewswire/ -- In February of 2000, the PCV-7 vaccine, Prevnar, was released. While vaccination rates vary and have improved over time, several studies have shown that the result of PCV-7 has had an enormous impact on the prevalence and proportion of the three common pathogens. Children vaccinated with PCV-7 are two-times less likely to have S. pnuemoniae and three-times more likely to have H. influenzae. It appears that PCV-7 has created a "pathogen shift." Given the recent changes in the microbiology of AOM, empiric treatment still focuses on the same 3 main pathogens, but the change in their frequency and resistance patterns should be taken into consideration.
However, a recent survey of 250 pediatricians conducted by DoctorDirectory.com revealed that of the physicians surveyed,
-- More than 62% of the respondents indicated that they believe S. pneumoniae is still responsible for the majority of AOM cases. -- More than 77% of the respondents indicated that they routinely target S. pneumoniae when treating empirically. -- Nearly 60% of the respondents still initiate empiric treatment with amoxicillin.
While it is apparent that the pathogens for AOM are changing, treatment patterns are not. This may explain why recent studies have shown that clinical failure while receiving antimicrobial treatment for acute otitis media (AOM) or persistence of infection detected within 30 days after treatment is completed, occurs frequently in children.
