Trusted Solutions Announces a New Program of Geriatric Care Management Services

NOVI, Mich., May 10, 2007 (PRIME NEWSWIRE) -- Trusted Solutions (tm), a division of Trinity Home Health Services, Michigan's premier provider of home health services, is proud to announce the grand opening of a new program for older adults and their caregivers called Trusted Solutions.
Trusted Solutions is a program of geriatric care management services dedicated to improving the quality of life for older adults and their families by connecting them to the resources, services and support that will enable them to remain safely in their homes.
The Geriatric Care Managers provided by Trusted Solutions are experienced healthcare professionals (Registered Nurses and Licensed Social Workers) with extensive knowledge of the aging adult and local resources. They can lighten the load for local and long distance caregivers by becoming the local care coordinator for aging parents. The program is currently available in the Southeastern Michigan Tri-County area.
Services are tailored to fit individual needs. It begins with a comprehensive in-home assessment of the older adult and the development of an individualized plan of care including recommendations to available community resources.
Care managers will provide a clear, objective picture of the current living situation and abilities for the older adult. They develop a personalized plan of care to address current and future needs. Geriatric care managers will be the local "eyes and ears" to provide professional guidance and support.
